The above represents the most common choices, however additional plans are available inside exchange for lower incomes.  Refer to plan summaries 
for complete benefit details.  Family members can choose different carriers and plans.  Final rates are dependent on ages of those enrolled as of 
January 1st.   Due to regulatory reasons, rates are the same regardless of agent or broker used.    Green Financial specializes only in the Washington
market and is the recommended Broker of the network.
a If enrolling children under age 19, ACA law requires pediatric dental/vision to be added (some plans inlcude in rates, some don't).  By law, all plans include 
preventic care, maternity and prescription drug coverage.  These are not options.
b Lifewise is an Exclusive Provider Option (EPO) not PPO.  Unlike Premera, no out-of-state network. Lifewise is NOT recommended in or out of exchange.
d Kaiser Permanente does not cover on-the-job injuries.  This is important for those who are self-employed and have
elected out of WA State L&I (workers comp) coverage particularly if you are in a trade such as construction, mechanic, installer, etc.
f If your income is below 300% of FPL, you must remove childen age 18 and under for an accurate quote.  You have two choices, 1) enroll them in the
Washington Medicaid program known as Apple Health or, 2) if you don't want them on Medicaid, you have the option to enroll them outside of the exchange
as indiviuals on their own contract.  Please call if questions on this.
* Out-of-Pocket Maximum includes deductible, copays, coinsurance and prescription copays
** Health Savings Accounts (HSAs) have a combined family deductible, other plans have max of two deductibles per person.  HSAs allow for tax
deductible contributions into an HSA bank account to be used for qualified medical, dental & vision expenses.  If just two people enrolling in HSA
plan (husband & wife) then better to enroll in separate plans to avoid combined deductible

We will need birth dates and social security numbers for all those enrolling.  
We will also need to know the plan you are choosing.
2)  Online Enrollment at WA Exchange
There are 17 electronic pages to work through, set aside about 30 minutes.
Please choose Green Financial from the broker field.  The premium cost is exactly
the same whether or not a broker is chosen.  We will be able to assist with any
questions or service needs if chosen as your broker.
